Insulin resistance is the engine behind most metabolic disease, and it can be elevated years before glucose rises. You also want a complete lipid panel that includes triglycerides and HDL, not just LDL and total cholesterol. The ratio of triglycerides to HDL is a strong marker of insulin resistance and small dense LDL particles, which are more atherogenic than the large fluffy kind. A 2014 study of 100 metabolic syndrome patients on rosuvastatin showed that treatment shifted LDL subfractions, but the standard lipid panel would not have caught it [1].

For liver health, add ALT and GGT. Nonalcoholic fatty liver disease is the most common liver condition in the developed world and is tightly linked to insulin resistance. A 2018 study of 212 type 2 diabetes patients found that ALT and GGT were significantly correlated with HOMA-IR [28]. The FIB-4 score, which combines age, platelets, ALT, and AST, can identify advanced fibrosis even when ALT is normal [20].

For kidney function, ask for cystatin C alongside creatinine. A 2013 NEJM study of over 90,000 people showed that cystatin C-based eGFR improved risk classification and mortality prediction compared to creatinine alone [7]. Creatinine is affected by muscle mass, diet, and hydration; cystatin C is not.

My call: fasting insulin, HOMA-IR, triglycerides, HDL, ALT, GGT, and cystatin C. That gives you the real metabolic picture. Confidence: high for the core tests (insulin, lipids, liver enzymes), moderate for cystatin C because it is not yet standard and may require out-of-pocket payment.
