The short version: statins can affect cognition, but the evidence is a mess of conflicting signals, and the system has not run the study that would settle it.

The evidence here is a pile of observational studies and small trials that point in opposite directions. The largest randomized trials, HPS and PROSPER, found no cognitive difference between statin and placebo over years of follow-up [3]. The Cochrane review of four dementia trials found no benefit from statins for treating Alzheimer's [4]. That sounds clean, but it is not the whole picture.

Case reports document real cognitive impairment that reverses when the drug stops. One review of 60 cases found patients reporting brain fog and memory loss that cleared within days to weeks of discontinuation [7]. A 2025 cross-sectional study of 23,220 statin users found short-term use linked to lower global cognition, though the effect disappeared after eight years [15]. A 2026 organoid study showed atorvastatin directly weakens brain blood vessels by stripping VE-cadherin from endothelial cells, a mechanism that could impair oxygen delivery and cognitive function [21]. That is a concrete biological pathway for harm, not a statistical artifact.

The observational meta-analyses claiming protection (30% lower Alzheimer's risk) are funded by no one in particular but suffer from healthy-user bias: people who stay on statins are healthier in ways that protect cognition [15]. The one randomized trial that tested statins for Alzheimer's treatment found no benefit [4]. The protection signal comes from epidemiology, not from experiments.
