What symptom-specific scores or tests are best for tracking improvement?

The short version: symptom-specific validated scales and objective biomarkers both track improvement, but the evidence is thin on which combination best predicts recovery trajectory.

The evidence retrieved shows that several validated scales are used to track specific symptoms in long COVID. The mMRC dyspnea scale and Fatigue Assessment Scale (FAS) were used in a 2023 prospective cohort of 669 adults to predict long-COVID risk at 90 days [3]. The SF-12 health survey, HADS for anxiety and depression, and mMRC were used in a 2025 longitudinal study of 50 adults to track improvement after an adapted physical activity program [4]. That study found large effect sizes: SF-12 global score rose from 35.2 to 48.6 (eta²=0.68), HADS-A fell from 10.2 to 6.1 (eta²=0.62), and dyspnea prevalence dropped from 86% to 52% [4]. The DePaul Symptom Questionnaire was used in a 2022 cross-sectional survey of 299 adults to classify PASC severity and found that 74-89% of severely impaired participants met ME/CFS criteria [16].

Objective biomarkers also correlate with symptom severity. A 2025 study of 120 recovered patients found that IL-6, CRP, and MDA levels correlated with fatigue scores (r=0.69, 0.65, 0.61 respectively) [11]. A 2025 study of 14 long-COVID patients with cardiovascular symptoms found persistent mitochondrial dysfunction in monocytes, with 80% mtDNA depletion and blunted oxidative stress response [9]. The 6-minute walk test and DLCO were used as objective measures in a 2026 study of 1011 hospitalized patients, with dexamethasone associated with better DLCO (76.9% vs 69.9% predicted) and longer walk distance (435m vs 405m) [6].
