The short version: fatty liver disease is often silent, but when it speaks, the symptoms are vague and the real danger is the progression you cannot feel.
The evidence confirms your hypothesis. NAFLD and alcoholic fatty liver disease are frequently asymptomatic, and when symptoms do appear, they are nonspecific: fatigue and right upper quadrant discomfort are the most common complaints [2, 8]. The 2019 ILTS consensus guidance describes fatigue as a key symptom, and the 2021 Liv.52 DS trial found that 75% of its 60 NAFLD patients had hepatomegaly at baseline, which is the physical exam correlate of that discomfort [2, 3]. The real risk is that the disease progresses silently through fibrosis to cirrhosis and hepatocellular carcinoma, with liver-related mortality in NASH estimated at 15.44 per 1000 person-years [8]. The evidence here is consistent and well-established, but it is also thin on the specific symptom profile because the condition is defined by its lack of symptoms until advanced stages.
