Are there any reports of vaccine flare-ups or autoimmunity after tetanus?

The short version: yes, there are documented reports of autoimmune and inflammatory events after tetanus vaccination, but the evidence is thin, old, and mostly case-level, not population-level proof.

The strongest documented signal is Guillain-Barré syndrome. A population-based study found no increased GBS risk above baseline after tetanus-toxoid vaccines, yet the IOM still concluded the evidence favored a causal relationship, based on two patients whose nerve inflammation returned after repeat tetanus shots [23]. That is a rechallenge pattern, the closest thing medicine has to proof of causation outside a trial. It is not nothing.

The other documented event is in children with fibrodysplasia ossificans progressiva, a rare genetic condition where muscle turns to bone. Six of 23 children had a flare-up within six hours of a DTP shot, and the injections caused permanent bone formation at the injection site [16]. That is a specific vulnerable population, not a general one, but it shows the tetanus toxoid can trigger a serious inflammatory cascade in the right body.

EventEvidence typeFinding
Guillain-Barré syndromeCase reports + population study2 rechallenge-positive cases; no population-level excess [23]
Heterotopic ossification in FOP childrenCase series, 23 patients6 flare-ups within 6 hours; permanent bone at injection site [16]

The evidence base is old, mostly from the 1990s and 2000s, and no recent surveillance from Nova Scotia or Canada appears in this retrieval. The mechanism is plausible: tetanus toxoid is a strong adjuvant-driven antigen that can push the immune system toward inflammation and molecular mimicry. But the honest gap is that no modern, well-powered study has tracked autoimmune events after tetanus boosters in adults.

My call: autoimmune flare-ups after tetanus vaccination are documented but appear rare and mostly in susceptible people. Confidence: low — the evidence is case-level, old, and the population-level studies are too weak to rule out a small real risk.
