Can you dissolve small gallstones with supplements?
The short version: bile acid supplements can dissolve small cholesterol gallstones, but the success rate is modest and recurrence is common.
The evidence for this comes from studies going back to the 1970s, all funded by academic or clinical institutions, not by a supplement manufacturer. That is a point in their favor. The 2025 cross-sectional study of 100 patients found that after at least 6 months of ursodeoxycholic acid (UDCA), only 18% had complete stone dissolution and 36% had partial reduction [12]. A 1982 trial showed that higher doses (12 mg/kg/day) worked better than lower ones, and that stones smaller than 5 mm dissolved in about 72% of patients while stones over 10 mm dissolved in only 19% [3]. The mechanism is straightforward: UDCA changes the bile's cholesterol saturation index, making it less likely for cholesterol to stay solid [1, 4]. But there is a catch. A long-term follow-up study found that even after complete dissolution, about half of patients had their stones return within 11 years [6]. And stones that contain calcium carbonate, which is common, resist dissolution entirely [7].

My call: for a person with small, radiolucent (not calcified) cholesterol stones and a functioning gallbladder, UDCA is a reasonable option with a low side-effect profile, but the chance of full success is about 1 in 5 and the stones may return. Confidence: moderate.
