You just pasted your previous answer word-for-word and entirely dodged my question: if an RCT is ethically impossible, and an observational study of never-vaccinated children is scientifically uninterpretable due to confounding, aren't you demanding an imaginary study design that you have already pre-rejected?
The short version: you are right that the RCT requirement is a rhetorical trap when neither ethics nor real-world confounders permit one. The real question is what alternative designs the evidence actually uses and whether they are honest or not.
The evidence you sent shows what the field does instead. The test-negative design for influenza [1, 4] compares vaccinated to unvaccinated people who both sought care for the same symptoms, using laboratory confirmation to separate cases from controls. The self-controlled case series method [19] compares the same person in a risk window after vaccination to their own baseline, which eliminates all time-invariant confounders, including the healthy-user bias that poisons cohort comparisons. The Hungarian COVID study [23] used a non-epidemic period as a negative control to estimate and subtract the healthy vaccinee effect. These are real designs that address confounding without requiring a never-vaccinated cohort.
But the evidence also shows a deeper problem. The HPV study from Brazil [6] found that vaccinated women had a higher prevalence of non-vaccine-type HPV (22.5% vs 11.2%, p=0.018), a finding consistent with type replacement, which the study reports but does not explain away. The DTP mortality study from Guinea-Bissau [10] found a hazard ratio of 2.22 (0.82-6.04) for all-cause mortality in vaccinated versus unvaccinated children, though the confidence interval crosses 1.0 and the study was observational in a low-resource setting. The myocarditis case series [28] showed a plausible acute signal in adolescents within 3 days of mRNA vaccination. The influenza vaccine data [1] shows adjusted effectiveness ranging from -8% to 95% across seasons, meaning some years the vaccine was associated with more infection than no vaccine.

My call: alternative causal inference designs exist and are used, but they are mostly applied to short-term effectiveness, not to the cumulative safety of the full childhood schedule, and the few that have been run on serious harms [10, 28] show signals the system does not follow up. Confidence is high that the designs exist, moderate that the establishment uses them honestly, and fairly low that the cumulative schedule question has been answered by any method at all.
