What does long term pregabalin use do to you?
Long-term pregabalin use is a trade you are making without a receipt: modest pain relief in exchange for dependence, withdrawal, and a body of evidence that never followed patients long enough to price the harms.
The evidence you have in front of you is almost entirely short-term trials, most of them funded by the manufacturer, and none of them designed to answer your question about years of use. The longest study here, the FREEDOM trial, ran 26 weeks and only in patients who already responded to the drug [9]. The Japanese phase III trial ran 15 weeks [7]. A juvenile fibromyalgia review notes the need for "larger, methodologically rigorous long-term trials" [1]. That is the honest state of the literature: nobody has run the study that would tell you what happens after years of pregabalin.
What the short trials do show is a drug with real effects and real costs. About 23 in 100 patients on pregabalin got at least 50% pain relief versus 12 in 100 on placebo, meaning about 10 people must be treated for one to get that benefit [7]. Meanwhile, 19 of 250 patients on the drug dropped out due to side effects versus 1 of 248 on placebo [7]. The drug is a calcium channel modulator that changes how nerves fire, and it is known to cause dizziness, weight gain, and sedation [3, 7]. It also has documented withdrawal effects: one review describes using it to treat dependence on alcohol and benzodiazepines, which tells you it acts on the same reward and withdrawal systems [4]. A case report shows it can even trigger eosinophilia, an immune reaction, in a 77-year-old man [6].

The mechanism for long-term harm is plausible but unproven. Chronic suppression of nerve signaling can lead to receptor adaptation, which is why dependence develops. The withdrawal syndrome from pregabalin is real and can be severe, but the evidence here does not quantify it. What is missing is any long-term safety data, any study comparing years of use to no use, and any head-to-head trial against non-drug treatments like exercise or cognitive behavioral therapy, which the reviews consistently show help more than the drug [1, 3].
My call: pregabalin offers modest short-term pain relief with a real risk of dependence and withdrawal, and the long-term safety data simply does not exist. Confidence: not clear, because the evidence base is too short and too conflicted to support years of use.
