What MRI findings confirm optic neuritis vs NAION?

The short version: MRI can distinguish the two, but the evidence base is thin and mostly old.

The key finding is optic nerve enhancement after gadolinium injection. In optic neuritis, the nerve itself lights up on fat-suppressed T1-weighted images, especially in the intraorbital segment [1, 4]. This enhancement is sensitive but does not predict how well vision will recover [4]. In NAION, the nerve does not enhance. The swelling is in the optic disc and the tissue just behind it, not in the nerve sheath or the nerve itself [3, 7]. A 2019 review confirms that NAION shows no optic nerve enhancement on MRI, while optic neuritis does [3].

The evidence here is weak. The best study on enhancement is from 1992 with only 8 patients [1]. A 2002 study of 2 patients confirms the pattern but adds nothing about reliability [4]. No head-to-head trial has ever compared MRI accuracy for these two conditions in a large, blinded sample. The 2019 review is expert opinion, not a diagnostic trial [3]. The claim that enhancement distinguishes them is plausible and widely repeated, but the published evidence supporting it is surprisingly small.
