Are there proven weight-loss strategies that work while on pregabalin?
The short version: no weight-loss strategy is proven to work specifically against pregabalin's weight-gain effect, because the drug was never studied that way.
The evidence shows that weight gain is a known, dose-related effect of pregabalin. In the one-year anxiety study, 24.4% of patients reported weight gain [3]. In the diabetic neuropathy trial, 11% reported it [5]. The fibromyalgia and epilepsy trials list it as a common adverse event [1, 2, 4]. But none of these studies tested a weight-loss intervention alongside the drug. No retrieved study compared diet, exercise, or any other strategy in people taking pregabalin versus people not taking it.
The mechanism matters here. Pregabalin binds to calcium channels in the brain and alters neurotransmitter release. It can increase appetite, slow metabolism, and cause fluid retention (peripheral edema was reported in 13% of patients in one trial [5]). A strategy that works for ordinary weight gain may not work against a drug that is actively pushing the body to store weight and hold fluid. The evidence simply does not address this.
What the evidence does show is that the weight-gain effect is dose-related. Higher doses produce more weight gain [1, 2]. So the most effective weight-control strategy supported by the evidence is to use the lowest effective dose. If you are tapering off, the weight-gain pressure should ease as the dose drops, though withdrawal risks mean you should taper slowly.
My call: no weight-loss strategy is proven to work specifically against pregabalin-induced weight gain. The evidence supports using the lowest effective dose and tapering when possible. Confidence: high that the evidence does not answer this question.
