Which exact lab panel should I ask for, and why each one matters?
The short version: you need a fasting insulin, not just glucose, and a lipid panel that includes triglycerides, not just total cholesterol. The rest depends on what your body is telling you.
A comprehensive metabolic assessment starts with what most GPs already order: a fasting glucose, a basic metabolic panel (BMP) for kidney function and electrolytes, and a lipid panel. But those miss the real story. The key addition is fasting insulin, which lets you calculate HOMA-IR, a direct measure of insulin resistance. Insulin resistance is the engine behind most metabolic disease, and it can be elevated years before glucose rises. The NHANES data shows median fasting glucose in adults 40-59 is only 99 mg/dL [1], well within the normal range, while insulin resistance is already driving disease.
You also want a complete lipid panel that includes triglycerides and HDL, not just LDL and total cholesterol. The ratio of triglycerides to HDL is a strong marker of insulin resistance and small dense LDL particles, which are more atherogenic than the large fluffy kind. A 2014 study of 100 metabolic syndrome patients on rosuvastatin showed that treatment shifted LDL subfractions, but the standard lipid panel would not have caught it [11].
For liver health, add ALT and GGT. Nonalcoholic fatty liver disease is the most common liver condition in the developed world and is tightly linked to insulin resistance. A 2021 trial of 90 NAFLD patients found that ALT dropped by 17 to 27 U/L across treatment groups after 12 weeks [18]. The FIB-4 score, which combines age, platelets, ALT, and AST, can identify advanced fibrosis even when ALT is normal.
For kidney function, ask for cystatin C alongside creatinine. A 2025 Swedish cohort study of 2,279 older adults showed that impaired kidney function (eGFR below 60) was associated with higher levels of neurodegeneration biomarkers like NfL, but did not independently increase dementia risk over 16 years [19]. Creatinine is affected by muscle mass, diet, and hydration; cystatin C is not.
My call: fasting insulin, HOMA-IR, triglycerides, HDL, ALT, GGT, and cystatin C. That gives you the real metabolic picture. Confidence: high for the core tests (insulin, lipids, liver enzymes), moderate for cystatin C because it is not yet standard and may require out-of-pocket payment.
