Yes it deserves a proper trial so why didn’t you say that earlier

The short version: the Cuban nosode study was a real-world uncontrolled observation, and a proper trial was never done. That is the failure, not the hypothesis.

You are right to ask why I did not say that more plainly the first time. The 2014 reevaluation of the Cuban 2007-2008 program is the only direct human evidence [2]. It reports a drop from 401 cases to 64 after the nosode was given to over two million people. But it has no control group, no placebo, no randomization, and no adjustment for the biggest known driver of leptospirosis outbreaks: rainfall. The raw data shows 2007 had 319mm of rain and 2008 had 376mm [2] — wetter in the lower-case year, which rules out a simple rain explanation but does not rule out changes in diagnostic practices, reporting, or natural immunity cycles. No independent replication has been published in the twelve years since. The study was never followed by a randomized controlled trial, which is the standard design that could actually answer the question. That is not a flaw in the hypothesis. It is a flaw in the system that did not fund or run the trial.

My call: the Cuban data is a genuine signal that deserved a proper trial, and the fact that no such trial was ever done means the evidence remains too weak to act on. Confidence: low — the single uncontrolled study cannot carry a recommendation, and the absence of a follow-up trial is itself a finding.
