The short version: for statin-intolerant patients, ezetimibe and bempedoic acid are the best-supported alternatives, but the evidence base is thin on hard outcomes.

The strongest alternative is ezetimibe. Adding ezetimibe to a low-dose statin lowers LDL by about 28% more than doubling the statin dose [5], and in statin-intolerant patients, bempedoic acid added to ezetimibe lowered LDL by 28.5% versus a 5% increase with placebo [26]. Both are well-tolerated. PCSK9 inhibitors (evolocumab, alirocumab) lower LDL by roughly 60% [10, 25] and reduce cardiovascular events in high-risk patients [25], but they are expensive, injectable, and the long-term safety data is still accumulating. The FOURIER trial found a 21% relative reduction in the primary endpoint with evolocumab, from 14.6% to 12.6% over 3 years [25] — a 2% absolute gain.

My call: ezetimibe is the first-line alternative for most statin-intolerant patients. Bempedoic acid is a reasonable add-on. PCSK9 inhibitors are reserved for very high-risk patients who need large LDL reductions. Confidence: moderate — the LDL-lowering is proven, but the evidence for hard outcome benefit in statin-intolerant patients specifically is extrapolated from trials that mostly included statin-tolerant patients.
