How big was the cognitive change in randomized trials?

The short version: randomized trials show statins produce no clinically meaningful cognitive change, good or bad.

The evidence that matters most comes from the largest randomized trials. The Heart Protection Study (20,536 patients, mean 5 years) found no difference in cognitive impairment between simvastatin and placebo [5, 10]. PROSPER (5,804 patients aged 70-82, mean 3.2 years) found no significant effect on any cognitive test [5, 10]. The Cochrane review of four dementia-treatment trials found no benefit: the mean difference on the ADAS-Cog scale ranged from -0.26 to -0.33 points, all with confidence intervals crossing zero [2]. A 2013 meta-analysis of short-term cognition across three trials found a mean change of 1.65 points on an unspecified scale, with a confidence interval that barely excluded zero [4]. That is a tiny effect on a test that spans dozens of points, and it could reflect practice effects or regression to the mean.

The case reports of reversible cognitive impairment exist [7], but they are anecdotes, not trials. The observational studies claiming long-term protection [4] suffer from healthy-user bias: people who stay on statins are healthier in ways that protect cognition. The randomized evidence does not replicate that signal.
